    • how does alli work?

alli works by inhibiting some of the body’s absorption of dietary fat. Taken at meal times, it works by blocking the absorption of about 25% of fat in the foods you eat. Because of the way it works, alli must be used in conjunction with a reduced-calorie, low-fat diet containing about 15 grams of fat per meal. Eating a meal with too much fat while taking alli can result in bowel changes, such as loose stools, more frequent stools that may be hard to control, or gas with oily spotting. These changes, called treatment effects, may occur in the first weeks of treatment, are not harmful and can generally be managed by following a reduced-calorie, low-fat diet that does not exceed 15 grams of fat per meal.

alli works in the gut; it is nonsystemically acting, which means it does not affect the central nervous system (CNS) and has no effect on the heart or the brain. Some weight-loss drugs and supplements that are systemically absorbed can be associated with CNS side effects (e.g., seizure, stroke and anxiety) or systemic side effects (e.g., heart palpitations, elevated blood pressure and heart problems). alli, a nonsystemic weight-loss aid, is not associated with these side effects, and its safety and efficacy are well established.

    • what are the side effects of alli?

Most side effects are related to the way you take the product and how much fat you consume when taking alli. Not everyone experiences GI side effects (or “treatment effects”), but they can be manageable when you follow a reduced-calorie, low-fat diet.

All drugs can have side effects and you should check with your doctor if anything unusual or severe occurs when using any weight-loss product. In controlled trials, only about 5% of subjects on alli dropped out due to treatment effects. In fact, anecdotally, many users have told us that treatment effects served as a signal that helped them adopt healthier eating patterns.

    • who can take alli?

alli can be taken by overweight individuals age 18 or older who are ready to make the change to a reduced-calorie, low-fat diet. But, as with all medicines, there are some people who should not take alli or who need to check with their doctor first, so it is important to review the label before purchasing alli.

    • how is alli different from other weight-loss aids?

alli is not a dietary supplement. It is an FDA-approved weight-loss aid. Orlistat (the active ingredient in alli) has been extensively studied and safely taken by millions of people worldwide. Americans spend an estimated $33 billion on weight-loss products and programs that in some cases are unregulated and unsafe. The absence of a proven safe and effective OTC aid for weight loss has been a void in the nation’s arsenal of weapons to fight the overweight and obesity epidemic. Orlistat is the only weight-loss aid approved by the FDA to go over-the-counter. Orlistat can help people lose 50% more weight than with dieting alone. That means if you could lose 10 pounds with a diet, you could lose 15 pounds working with alli. Individual results may vary.

    • is there a benefit in losing 5% to 10% of your weight?

Obesity experts in the U.S. agree and widely available research demonstrates that moderate weight loss is beneficial. It is important to emphasize the health benefits of 5% weight loss and to change the perception of Americans who reject this as an insignificant amount of weight loss. Achievable, realistic weight loss of 5% can result in significant improvements in overall health, including decreased risk for heart disease. Slow, steady weight loss is the healthy way to reduce body fat mass. The alli program encourages the lifestyle changes that help achieve realistic goals of improved health and weight-loss maintenance.

    • the math behind alli does not seem to add up. If the drug in the alli capsule inhibits the absorption of about 25% of the fat that I eat, and I follow the plan, it would take me around 5 weeks to lose 1 pound. How does that equate to 50% more weight loss than dieting alone?

It is important to remember that alli capsules are not intended to do all of the work for you or to replace diet and exercise. alli is indicated to be taken with a reduced-calorie and low-fat diet.

Clinical studies have proven that people who followed a reduced-calorie, low-fat diet while taking alli capsules experienced 50% more weight loss than those who just followed the diet. alli capsules (Orlistat 60 mg) prevent the absorption of about 25% of the fat you eat and cause a calorie deficit in your diet. The actual amount of fat blocked by alli is dependent on the total amount of fat consumed. As discussed in the educational materials that are provided in the alli starter pack, individuals will have different total caloric daily needs based on their weight and activity levels.

In addition, individuals who used alli reported experiencing greater motivation. They were more likely to follow and maintain a low-fat diet while on alli. In addition, they were more likely to exercise.

The mealtime dosing schedule (one hour before, during, or one hour after a meal) makes it easy to remember alli capsules. To help you remember, we recommend making alli a part of your pre-meal routine. Once alli and the routine have been paired several times, the association will become automatic. Clinical studies show alli works best when you eat 3 healthy, low-fat meals a day, one of which is breakfast. If you skip a meal or you are certain your meal contains no fat, you can skip a dose.

The recommended dose of alli is 1 capsule with meals containing fat and no more than 3 capsules daily. alli should be taken within an hour of each meal containing fat. • safety and drug interactions

    • is alli safe?

alli has been approved by the FDA as safe and effective when taken according to label directions.

Taken at mealtime, alli works by blocking the absorption of ¼ of the fat in the foods you eat. alli is nonsystemically acting, which means it does not affect the central nervous system (CNS) and has no effect on the heart or the brain.

The safety and efficacy of Orlistat are both well established. A higher dose of Orlistat has been marketed as the prescription drug Xenical in the U.S. since 1999 and is supported by nine years of use in 145 countries. Orlistat experience in more than 25 million patient treatments and more than 100 clinical studies with more than 30,000 patients is unprecedented for a weight loss drug. One of these studies was the four-year landmark XENDOS trial conducted by its manufacturer, Roche. This study is the longest study ever conducted for a weight-loss drug.

    • does alli cause any heart racing or palpitations?

No. alli works in the gut; it is nonsystemically acting, which means it does not affect the central nervous system (CNS) and has no effect on the heart or the brain.

    • what is the rationale behind the label advice for patients with gallbladder disease, pancreatitis and kidney stones to speak with their doctor?

Since everyone has different GI transit times, the active ingredient in alli can actually remain in your system for up to 48 to 72 hours, mainly because gastric emptying time varies from person to person.

    • will alli interfere with birth control medications?

No. alli does not impact the efficacy of your birth control medication. • does alli interfere with statins (e.g., Lipitor®, Zocor®, Pravachol®, etc.)?

    • do alli and Zetia® work the same way? Can I take them both together?

Zetia reduces blood cholesterol by inhibiting the absorption of cholesterol in the small intestine. alli, on the other hand, works in the stomach, where it inhibits the action of gastric and pancreatic lipases. Since these two drugs have different mechanisms of action and work at different sites in the body, it is unlikely that there will be an interaction between the two.

    • is it OK for diabetics to use alli?

Ask a doctor before using alli if you are taking medicines for diabetes. There is no drug interaction between alli and diabetes medications; however, people with diabetes may have lower blood sugar levels and improved glycemic control after losing weight with alli. • can alli be taken with Benecol®?

Yes. There has been no interaction identified between the ingredient in Benecol (plant stanol esters) and Orlistat (the active ingredient in alli). On the alli program, you should aim for 15 grams of fat or less from each meal. One serving of Benecol Spread or Benecol Light Spread contains 8 or 5 grams of fat, respectively. Therefore, patients need to be mindful of the contribution of the fat in Benecol to the total amount of fat in the meal.

  • treatment effects or adverse events

    • will alli affect my sleeping pattern?

No. alli is nonsystemic acting and won’t affect the central nervous system. You will not feel the same jitteriness or palpitations from the drug.

There has been no evidence in the clinical trials that indicates that it causes a problem. If you experience sleep problems, please see your doctor.

    • does taking fiber with your diet help to reduce treatment effects?

There is no conclusive scientific evidence that fiber will reduce the treatment effects of alli, although fiber has many benefits to your body. However, eating a reduced-calorie, low-fat diet will help to manage treatment effects.

    • what about treatment effects?

Treatment effects are bowel movement changes that are most commonly caused by eating meals with too much fat while using alli capsules. Such effects may include oily spotting, loose stools and more frequent stools that may be hard to control.

alli works by blocking the absorption of a quarter of the fat in the foods you eat. This undigested fat passes through the body naturally and it is not harmful. In fact, you may recognize it in the toilet as something that looks like the oil on top of a pizza.

Eating a low-fat diet that consists of 15 grams of fat per meal on average can lower the chance of experiencing these treatment effects.

Additionally, the alli program can help you reduce the likelihood of having treatment effects. For example, when you purchase alli, you receive educational booklets, which help you transition to a healthy lifestyle. You’ll also receive access to myalliplan™, an individualized, online support program created by weight-loss experts, where you can get recipes and meal plans for low-fat meals.

    • how do I report an adverse event from taking alli capsules?

To report an adverse event or side effect as a result of using alli capsules, please call 1-800-671-2554, M-F, 9 a.m. – 4:30 p.m. ET so we can give you personalized attention. In the event of a medical emergency, please contact your doctor, local emergency facility, or local poison control center. To report an adverse event directly to the FDA, please call 1-800-FDA-1088.

  • dietary fat and alli

    • what is dietary fat?

Fat has more than twice the calories per gram than carbohydrates or protein (9 calories per 1 gram of fat versus 4 calories per 1 gram of carbs or protein). Dietary fat is the most concentrated source of food energy. Dietary fats are composed chiefly of triglycerides, such as the fat in meats, milk products and vegetable oils. Research attributes health risks to certain kinds of dietary fats. For example, it is believed that saturated fats (e.g., animal fats and butter) may be more dangerous to your health than polyunsaturated (e.g., corn and soybean oils) and monounsaturated fats (e.g., canola and olive oils), which may have a beneficial impact. All fat types, however, have the same number of calories and are affected by alli in the same way.

    • why is it important to target dietary fat?

Intake of excessive calories can lead to weight gain. Because dietary fat is calorie dense (2 times as much per gram than other nutrients), it can quickly lead to excessive calorie intake and weight gain. Weight gain is one of the major risk factors for conditions leading to heart disease, including high blood pressure, high cholesterol, and diabetes.

    • what can I do to help determine the amount of fat per meal?

Use the reference booklets in the alli starter pack. The pack includes: Keys to Successful Weight Loss (“Read Me First”), Welcome Guide (“Hi”), Quick Facts Cards, Companion Guide, Healthy Eating Guide, Calorie & Fat Counter, and Daily Journal. The alli program gives you access to a comprehensive support program, including tools to help evaluate dietary fat content.

    • what if I consume a diet containing more than 15 grams of fat per meal?

alli is recommended for use 3 times daily with meals containing fat. Meals should contain 15 grams of fat or less. Dietary fat consumption beyond the recommended 15 grams of fat per meal may result in an increased incidence of treatment effects, such as loose stools, more frequent stools that may be hard to control, or gas with oily spotting. These changes may occur in the first weeks of treatment, are not harmful and can be managed by following a reduced-calorie, low-fat diet with no more than 15 grams of fat per meal. In comparative trials, only about 3% of subjects on alli dropped out due to treatment effects. In fact, anecdotally, many users reported that treatment effects served as a signal that helped them adopt healthier eating patterns.

    • does alli discriminate between types of fat, or does it prevent the absorption of both good and bad fats?

alli does not discriminate between types of fat but prevents the absorption of about 25% of fat consumed. But remember, eating too much fat, whether it’s “good” or “bad” fat, can contribute to weight gain and possibly result in treatment effects. You should stay within your fat gram goal for each meal.

    • what about supplements such as vitamin E and flaxseed oil? Should I discontinue or limit them?

We recommend you take a multivitamin (which contains vitamins A, D, E, K and beta-carotene), at bedtime, while taking alli. If your doctor has recommended taking other supplements, take them as recommended by your doctor. But, remember that these and some other supplements may contain fat, and the supplement will count toward your fat intake targets.

  • obesity and BMI

    • what is obesity?

Obesity can be defined as an excess accumulation of body fat sufficient to endanger health. It is a disease (not a personal weakness) that requires long-term treatment and management to achieve and sustain weight loss.

Experts agree that the primary cause of obesity is the excessive intake of calories, generally as fat. Obesity results from an imbalance between energy intake and energy output (excess calories consumed, insufficient or decreased physical activity). The correlation between energy intake, expenditure and resulting body fat distribution varies by individual, which is what makes the reasons for this imbalance so unclear. In general, it is easiest for the body to store excess calories if they are consumed in the form of fat. It is harder for the body to convert excess carbohydrates or protein into fat.

Other factors which are thought to contribute to obesity are genetic, environmental, socioeconomic, cultural and psychological.

    • how is obesity measured?

The most widely accepted measurement of obesity is known as the Body Mass Index (BMI).

BMI is calculated by the weight (kg) divided by the height in meters squared.
The BMI is graded to indicate the degree of risk to health.
      BMI < 18.5 (underweight)
      BMI 18.5-24.9 (desirable weight)
      BMI 25-29.9 (overweight)
      BMI 30-40 (obese)
      BMI > 40 (severely obese)

Other methods of measuring obesity include:

      • waist circumference – measuring the distribution of abdominal fat, which is well correlated with the risk of developing cardiovascular diseases.
      • height-weight ratio – using an appropriate measurement of frame size – small, medium, large – height/weight tables can be used to estimate ideal weight.
      • measuring percent of body fat – using skinfold test and/or the body impedance method.
    • why does obesity need to be managed?

Obesity is associated with an increase in serious health risks including:

      • elevated blood pressure and cholesterol levels
      • diabetes (non-insulin dependent diabetes mellitus)
      • some forms of cancer
      • osteoarthritis
      • gallstones
      • respiratory complications, such as increased difficulty in breathing while asleep
      • reduced quality of life

If you have a question about obesity in your personal health, check with your doctor.

In order to help ease you into myalliplan, the alli 1-2-3 Start Plan was created to give daily tips and lessons on how to begin taking alli and to introduce you to myalliplan. This plan is a 10-day program that you will complete before your first check-in.

  • miscellaneous

    • how much does alli cost?

Generally, the cost is roughly what one might expect to pay for a bag of chips and a can of soda every day – the equivalent of an afternoon snack.

    • how much exercise do you recommend while taking alli?

You don’t have to hit the gym or hire a personal trainer to make a real difference in your health. Studies show that moderate exercise – such as walking for 30 minutes every day – can reduce your risk for disease.

The great thing about exercise is its cumulative effect – you don’t have to do it all at once. You can add it to your activities throughout the day by sneaking in a little extra walking to the things you already do.

What’s important is including activity regularly in your routine. Start small and work your way up. Consistency is your goal.

    • what is the difference between the “Are you losing it?” book and “The alli Diet Plan”?

The “Are you losing it?” book educates people about a realistic mind-set, healthy weight-loss strategies and a sensible approach to losing weight and keeping it off. The book is a tool to expose myths and obstacles that discourage weight loss, give advice on lifestyle and behavioral changes, provide low-fat menu ideas and offer physical activity tips. The book also includes an overview of alli, the only FDA-approved, over-the-counter weight-loss aid. It is sold near the pharmacy counter of participating stores.

“The alli Diet Plan” contains recipes and tools helpful while you follow the alli program. The book moves dieters through three phases, each with the same caloric intake but different levels of fat content. The tools include more than 200 delicious, alli-friendly recipes, including no-cook, quick and easy and prepare-ahead options; a restaurant guide with 100 restaurants; a low-intensity, fat-burning fitness program to help you achieve results; and 300 daily menus/meal plans.

    • i have looked at my local store, and I can’t find alli on shelves. What should I do?

We are sorry to learn that you have found it difficult to locate alli in your area. Please be assured that we are working diligently to ensure that alli is available as quickly as possible.

Participating grocery, drug and mass retailers, as well as some online retailers, sell alli. If you cannot find alli in one store, you should check another local retailer or go online.

Additionally, you might wish to order our product online from leading retailers. They can ship the product directly to you.

    • I have been reading “The alli Diet Plan” book. I noticed that there are differences between the information provided in the book and the information in the alli starter pack books. Why is that?

“The alli Diet Plan” book was written by Dr. Carolyn Apovian and published by Meredith Publishing. The book has a three-phased approach. The approach moves from Phase 1 (15% of your calories from fat) to Phase 3 (30% of your calories from fat). This plan was independently developed by Dr. Apovian to minimize treatment effects.

“The alli Diet Plan” is compatible with alli. It does follow the same calorie targets as the information provided in the alli materials, but the Phase 1 and Phase 2 fat targets are slightly lower.

    • why are you recommending people take alli with a multivitamin? Does it deplete vitamin levels?

It’s always sensible advice to take a multivitamin when you are on any kind of weight-loss program. alli works by preventing absorption of about 25% of fat, and it will also minimally inhibit fat-soluble vitamins to the same extent. The fat-soluble vitamins are vitamins A, D, E, K and beta-carotene.

We recommend that you take a multivitamin with alli, once a day at bedtime.

    • what are the different sizes that alli is available in?

alli is available in four sizes: 60 ct., 90 ct., 120 ct. and 150 ct. The 60 ct., 90 ct. and 150 ct. are all starter kits. This means that they contain all of the in-pack materials. The 120 ct. is a refill pack and only contains the Companion Guide and the Read Me First guide. Additionally, the 150 ct. is available at club/warehouse stores.

The safety and efficacy of alli are well established, and clinical trials with Orlistat (the active ingredient in alli) have consistently shown it to be more effective than diet alone. It can aid weight loss in individuals who are committed to losing weight, regardless of ethnicity.

The population at large, including African Americans and Hispanics, has been shown to achieve weight loss when committed to the program.
